DOCUMENTATION REQUIREMENTS FOR CLAIMS UNDER
THE INCOME PROTECTION POLICY

A. Unemployment Claims

1. Copy of passport, valid visa and Emirates ID
2. Notice of termination from the Employer showing the reason for termination
3. Salary slips for the 3 months preceding date of termination or Salary Certificate
4. Copy of Labour Contract
5. Any other documents as may be required by us to validate the claim

B. Accidental Death Claims

1. Original Death Certificate (returnable immediately after verification). In case of death occurring outside the UAE, the Death Certificate should be attested by the relevant authority in the country of death, UAE Embassy in the country where the death occurred and the Ministry of Foreign Affairs in UAE
2. Accident, Forensic, Medical Reports related to the death
3. Copy of passport, valid visa and Emirates ID
4. Copy of Entry and Exit stamp in the Passport (for death outside UAE).
5. Copy of Emirates ID
6. Any other document as may be reasonably required by us.

C. Hospitalisation Claims

1. Copy of passport, valid visa and Emirates ID
2. Medical Certificate/ Reports from the treating Doctor or Hospital/Clinic.
3. Discharge summary from the treating Doctor or Hospital
4. Leave Certificate
5. Police Report (for Accident related cases)
6. Any other document as may be reasonably required by us.
